From overwhelmed to organized
Hospital discharge does not have to mean confusion. Here is what changes with personalized nursing care at home.
- Family overwhelmed
- Questions unanswered
- Caregiver anxious
- Family organized
- Caregiver prepared
- Clear next steps
HVPN may be a good fit if:
- You recently returned home from the hospital.
- You’re caring for a parent or spouse.
- You have questions about discharge instructions.
- You’re feeling overwhelmed by medications.
- You want one-on-one guidance from an experienced RN.
When another provider may be more appropriate
HVPN does not provide emergency care, same-day dispatch, nurse staffing, overnight or continuous nursing coverage, personal care aide services, or home health agency services, and does not replace the treating physician, hospital, hospice, or a licensed home care provider.
You do not have to sort through everything alone.
Coming home from the hospital or managing a complex recovery is rarely straightforward. Discharge instructions are dense. Medication schedules are unfamiliar. Families are managing a great deal at once, and the next step is not always clear.
Hudson Valley Private Nursing is an RN-led private nursing practice helping individuals and families receive personalized nursing care at home, provided personally by Andreen Robinson, MSN, RN, based on clinical appropriateness, provider orders where required, and patient needs.
